What is PDSA? When using the PDSA cycle, it's important to include internal and external customers; they can provide feedback about what works and what doesn't. The customer defines quality, so it would make sense to also involve them in the process when appropriate or feasible, to increase acceptance of the end result. If you're unsure about, who your customers are, you may want to create a customer chain to assist in identification.
The Plan Do Study Act cycle (PDSA cycle)
Assemble a team that has knowledge of the problem or opportunity for improvement. Consider the strengths each team member brings—look for engaged, forward-thinking staff. After recruiting team members, identify roles and responsibilities, set timelines, and establish a meeting schedule.
Describe what you want to accomplish in an aim statement. Try to answer those three fundamental questions:. You might have already answered the last two questions if you have performed a SWOT analysis. You may find it helpful to construct a swim lane map to visually describe your process. Creating a process flow or at least depicting the current process can be very useful. If your team runs into road blocks, you might have found where the problem is occurring—or maybe the right person for identifying a missing step is not at the table.
For example: If your objective is to maximize your staff's quality of work life, you might find evidence by surveying employees on workplace stressors. Write a problem statement to clearly summarize your team's consensus on the problem. For the problem in your problem statement, work to identify causes of the problem using tools such as control chartsfishbonesand work flow process maps e.
The end of the cause analysis should summarize the cause analysis by describing and justifying the root causes. Choose an alternative or a few alternatives that you believe will best help you reach your objective and maximize your resources.
PDSA Cycle & Continuous Educational Improvement
Try to account for risks you might face as you implement your action plan. Start to implement your action plan. Be sure to collect data as you go, to help you evaluate your plan in Stage 3: Study.Understand the evolution of these variations. The Plan-do-check-act cycle Figure 1 is a four-step model for carrying out change. Just as a circle has no end, the PDCA cycle should be repeated again and again for continuous improvement. Teachers also can access comparative data on the electronic database to identify trends. High-need students are monitored by a special child study team. Throughout the school year, if assessments show students are not learning as expected, mid-course corrections are made such as re-instruction, changing teaching methods, and more direct teacher mentoring. Assessment data become input for the next step in the cycle.
In this example, the "act" step is "standardization. Teachers share best practices in formal and informal settings. Keep It Simple Quality Progress Being able to discern the state of a process at a glance allows problems to be identified and addressed quickly.

As a temporary test, they planned to change screening at one of their clinics as of a certain date, to follow up with the clinic after a month to determine the impact on the process and to track patient outcomes to see if the change had any adverse effects.
Do: The change test was carried out in one clinic and related data was gathered on an ongoing basis to understand the results of the change. Study: Results from the test clinic were compared to performance objectives, and conclusions were reached. Act: This step consisted of determining if any further changes were needed to the new process based on the results from the clinic. This may involve further or expanded tests or deciding whether the change should become permanent.
The lessons from the each PDSA cycle were used to expand the change to more of the clinics in the system. PDSA is a cycle and often shown as a graphic such as:. Steps : The idea of PDSA is a cycle of steps, generally implementing a small change, learning from the results and repeating the cycle until the desired change is fully complete and sustaining. These record for each step: Plan - Describe the problem, the objective of the test and a description of who, what, when and where changes are involved.
Define the baseline or initial descriptive data to determine if the change is as intended. Do - Carry out the test, document results and any problems. Study - Analyze the results, compare them to the objectives and decide on next steps or if the planned changes should be revised or, if necessary, abandoned. Act - Modify or refine the change and plan for the next cycle.
